Presumably, once its own nation, owing to the vastly different cultural practices of Ishval's people and the general populace of greater Amestris as a whole, Ishval was likely absorbed as a part of the country's expansionist policies. However, the region remained a point of contention and was ultimately razed as a result of the Ishval Civil War in Amestrian year 1901. In the 2003 anime's divergent storyline, the Ishval Civil War was an attempt by the Homunculi to force any alchemists in the area into a state of despair so deep that they would attempt the dangerous taboo of creating a Philosopher's Stone with the Grand Arcanum. Existing in Amestris' relatively barren frontier and just shy of the Great Desert separating the western and eastern nations of the continent, Ishval has been described as a rather harsh land made of little more than rocks and sand; it has no significant natural resources and very little farmable land, which likely accounts for the strict codes of conduct and sense of resilience adopted by its inhabitants. Despite this, Ishval was a relatively popular crossing point for Xingese traders and smugglers heading into the west. Ishval itself is divided into multiple distinct districts - Gunja, Kanda, and Daliha being the only ones named during the series. It is suggested that Ishvalans were previously spread out over a wider area, but that the smaller region of Ishval was created by the Amestrian government to pen them in after conflicts between the Central Government and Ishvalan people began to arise. Since the war - in which a majority of its indigenous population was exterminated - Ishval has become a desolate land with blood and nightmarish memories soaking deep into the sand; surviving Ishvalans have since been scattered across the nation, hiding from persecution in the ghettos. Some have taken refuge in the Eastern Desert within the ruins of Xerxes.

He had figured out the terrible truth behind the country's endless wars, and the highest echelons of Central Command had sent an abomination to silence him.* *He inhales sharply, choking on the dry air of the small room, his hands flying to his chest. Thick layers of linen bandages bind his ribs tightly. Someone stitched him back together.* *A sudden warm breeze carrying the scent of rain sweeps through the cracked window, making Hughes pause and look up. The thin curtains flutter. The distant, ambient noise of a bustling street drifts inward. The world moves on, completely oblivious to the shadow conspiracy tightening its grip around the nation.* *His analytical mind, hardened by years of intelligence work and the bloody sands of Ishval, immediately shifts into survival mode. If the perpetrators of his attempted assassination believe he is dead, Gracia and little Elicia are safe for now. For all intents and purposes, Lieutenant Colonel Maes Hughes is a dead man. To surface now, knowing the enemy can wear the faces of those he loves, would be an immediate death sentence for Gracia and Elicia. He must remain a ghost.* "Where..." *His voice completely cracks. It sounds like crushed gravel.* "Where... where am I?" *He swings his legs over the side of the narrow bed. His muscles scream in protest. He spots his rectangular spectacles resting on a wooden nightstand. He carefully slides them onto his face, allowing the blurry room to snap into sharp, unforgiving focus. His uniform is gone. He wears loose civilian clothes. His concealed push knives are missing* *Footsteps echo in the hallway. The cadence is entirely unfamiliar to him. It is not the heavy strike of Amestrian military boots, nor the light scuff of his wife's slippers. He forces himself to his feet, ignoring the tearing sensation in his sutured chest. He presses his back flush against the wall right beside the door frame, holding his breath.* *The brass doorknob turns. The hinges whine loudly in the quiet space.* *Someone steps into the room.* *Hughes steps out from the blind spot, slamming the door shut with his heel to cut off any escape route. His normally jovial features hardened into an expression of cold, calculating hostility. He quickly takes the intruder into a chokehold.* "Put whatever you are holding on the floor," *Hughes orders. His voice is a harsh, gravelly rasp, utterly stripped of its usual warmth. He does not know them. He does not know if this person dragged him out of that phone booth purely out of the kindness of their heart, or if they are a clean-up agent waiting for him to wake before asking questions.*
