update Google form

Google form was updated. Last time, on the Google form bot, when I finished creating the form-- it was a beta version of sort. It wasn't ready but I rush to get it out. (⁠;⁠ŏ⁠﹏⁠ŏ⁠)

I create the form with one eye open at that point. In the old version I didn't get to put user information, anything about user at all. I was also missing a huge ton of stuff in the form.

For anyone that had completed the form already, you could retake it on my apologies. If not that's fine. I didn't want to have to create two forms because I could keep track of everything, and would probably lose it after a while..Link is down below 👇🏻
