An Omega's ears and tail comes out as soon as they feel safe during heat or when they are together with an Alpha. Alpha's can go into rot, it's rare but when they smell the heat pheromones of their mate it can come up. It's very intense amd they get like wild beasts— they can go on for days during sex when Alpha's are in heat.
